Reactive arthritis (Reiter’s syndrome)

Reactive arthritis (Reiter’s syndrome):

  • Presents after a GI or GU infection***
    • Diarrhea, N/V -> arthritis!
    • Stool culture: + Shigella
  • “Can’t see (conjunctivitis), can’t pee (urethritis), can’t climb a tree (arthritis).”
  • Circinate balanitis: inflamed glans penis
  • Keratoderma blennorrhagicum: rash on bottom of feet.
  • Affects young adults
  • Asymmetric arthritis: pain, swelling in 1 knee!
  • Enthesitis: pain at tendon insertion site
    • Palpation of patella evokes pain
  • Pt keeps knee in flexion due to the pain
  • Dactylitis: (sausage digit) inflammation of an entire digit and can be painful.
  • Effusion present:
    • Fluid aspiration: WBC 4,000
  • Dx: Clinical.
  • Tx: NSAIDs (self-limited arthritis)
